 How does the Health plan work?
Each Family Care member receives preferred patient prices when he/she uses a participating provider.

 When does my membership become effective?
HealthCare - USAHealthcare will send a complete welcome packet within about 10-15 days of joining. If you pay by e-check there may be a 7 day delay in processing your welcome packet.
DentalCare - Coverage is effective the from the day of enrollment. For example, if you enroll in the plan on the 7th day of the month you will be charged a prorated fee for that month and then charged on the 1st of the month after that (if a monthly plan). If you need dental services immediately you will have full use of the plan within 48 hours of enrollment. (Remember, there are no waiting periods and pre-existing conditions are covered immediately.)

Call a participating dentist for an appointment and be sure to mention that you are a member of the DenteMax Dental plan. Your permanent card will arrive in 10-15 days.

 Who can qualify as a dependent under my plan?
The individual + 1 & family plans include 1 dependent & the entire household respectively. However,  dependent children until the age of 19, and dependent children until age 23 who are full-time students are eligible for the Complete Care benefits.

 Is there a limit to how many dependents I can have on my plan?
No, there is no limit for dependents.

 How can I add or remove a household member from my plan?
Simple. Just call our toll-free number (1-877-552-0924) or email us at customerservice@usahealthcare.com stating the household member you wish to add or remove. Please do not forget to indicate your name and member ID number so we can process the request.

 How can I cancel my membership?
You may cancel your membership at any time by calling us at 1-877-552-0924.

 Can I get a refund if I decide to cancel my plan?
In the event that you wish to cancel your membership during the first thirty days of your enrollment date, you will receive a full refund of your membership dues ,not including the enrollment fee. The enrollment fee is non-refundable.

 How many cards do we receive per family?
Each Family membership includes two permanent identification cards. The cards show the primary member's first and last name. Spouse and dependents are listed by first name only.

Preferred Medical Pricing
What if my doctor is not on the list?

That's easy! All you have to do is submit in writing the name, address, and any phone numbers of your doctor or dentist to our provider relations department via fax, email, or regular mail. We will invite him or her to become a Family Care provider! Note: This process is a courtesy to our members, not a guarantee that your provider will join Family Care or agree to give a discount.

 Can I use this service if I have medical insurance?
Yes. If your insurance policy pays a percentage of your medical bills, such as a standard 80/20 indemnity policy, then this service can be used. The discount will be applied to the entire bill, making your portion much less than 20%. And in many cases, your portion will be reduced to zero after your deductible with your insurance company has been met.

 How much do I have to pay when I go to the doctor?
The savings vary from one provider to another, however you will be expected to pay the discounted price while at the doctor. This plan is a "fee-for-service" plan.

 What is the name of this plan that the Dentist will recognize?
Although our site (domain) name is USAHEALTHCARE.com your Dentist will know us by the name DenteMax Dental. Please use "DenteMax" when referring to your program name.

 What exactly is a "Reduced Fee Program" ?
This is where a member will pay a monthly, quarterly or annual fee to receive significant savings from our network of Dentists. Members receive a "reduced fee" for Dental services compared to non-members. Once enrolled, a member can visit any of the 55,000 dentists, including all specialties and receive treatment at a reduced fee.  The savings range up to 50% off the dentist's customary fees.

 How is our dental program different from traditional insurance?
A reduced fee program is different in a number of ways from traditional insurance, but it also carries many valuable similarities.  Reduced fee programs remove much of the hassle and waiting that goes with enrolling in traditional insurance, unlike insurance we do not check for pre-existing conditions,  there are no complicated forms to fill out and send back and forth to any insurance company, there are no deductibles to worry about, and enrollment is available to anyone; at any age, regardless of your current health.

You will receive the same quality and treatment as you would with traditional insurance plans.

 Is the USADENTALCARE program considered insurance?
The program we provide is a discount program or reduced fee program. It is not considered insurance. When you visit a participating provider, you will receive discounted fees as listed in the Schedule of Benefits.

 What is a participating provider?

 What are the limitations of the Cigna Dental Network Access program?
Our programs have NO limitations on coverage in terms of yearly maximums.  However you must use a dentist in the Cigna Dental Access provider network in order to receive the discounted fees.  All DenteMax providers are carefully screened, credentialed, and selected in order to secure that you are given a choice of the finest dentists in your area.

 How does this service work?
After choosing a participating vision location, simply present your card when you visit the provider. The discounted rates will automatically be computed.

 How do I use this service?
After you have received your card, all you do is present it to one of the participating pharmacies. You will receive the lower rate for your prescription(s) at that time. There are no claim forms to fill out for reimbursement. Savings is applied at the time of service.
