1. Accidental Death & Dismemberment Coverage

This benefit is provided to all active members at MedNet Benefit’s expense as an added separate benefit at no additional cost to you.

2. $5,000 Medical Accident Coverage:
Pays for medical expenses resulting from an injury caused by a covered accident. This benefit is provided to all active members at MedNet Benefit’s expense as an added separate benefit at no additional cost to you.

Covered persons:
Primary member, Spouse and dependent children

Covered Expenses include:
Ambulance, Hospital room & board, Ancillary hospital expenses including operating room, lab tests & anesthesia, Registered nurse services for private duty nursing while confined in a hospital, X-ray expenses, Prescription drug expenses, Dental expenses including dental X-ray.
